The basic principles of the phenomenon "erection" have been known since the pioneering work of Kolliker, Eckhard and Langley in the nineteenth century. Nonetheless, under the influence of Freud, erectile dysfunction was predominantly at tributed to psychogenic factors. A more liberal perception of sexuality since the 1960s, the development of new and refined diagnostic techniques, and the expansion of basic research activity resulted in a new concept of erectile dysfunction, identifying arteriogenic, venogenic, endocrinologic or myopathic (cavernous smooth muscle dysfunction) factors. From this research other considerations such as autonomic innervation, cavernous endo thelial intactness or impaired neurotransmitter pool are being introduced into routine clinical assessment. A reevaluation of psychogenic etiology with a consequential new concept of psy chogenic impotence is on the rise. In this book the new concepts of basic knowledge on cavernous smooth muscle function and its supraspinal, spinal, and local control; the new diagnostic approaches in psychogenic and autonomic factors; and the new developments of reconstructive therapeutic options for the patient have been tied together. Outstanding and internationally renowned experts in the field of erectile dysfunction have given detailed insight into the latest basic and clinical developments. Well-established diagnostic and thera peutic techniques are presented by experienced colleagues. We hope that this book will help the reader to get an overview of the current concepts of erection. Furthermore, we hope that international collaboration in basic and clinical research in the field of erectile dysfunction will render an update necessary in the near future.

This concise guide to the often overlooked association between erectile dysfunction and hypertension/cardiovascular disease covers a wide range of aspects of importance to the clinician. It examines the impact of antihypertensive drug therapy on erectile function and explains how the management of erectile dysfunction in hypertensive patients depends on a variety of factors. Different treatment approaches are described, including lifestyle modification, PDE-5 inhibitors and other novel agents and behavioral therapy and helpful therapeutic algorithms are presented. A further focus of the book is the potential role of erectile dysfunction as an early diagnostic indicator of asymptomatic coronary artery disease and a prognostic marker for cardiovascular events. In addition, key background information is supplied on epidemiology and pathophysiology, and the significance of erectile dysfunction in different patient groups, such as the elderly and those with chronic kidney disease, is examined. Erectile dysfunction is a major public health problem affecting more than ten percent of the general male population and is now considered to be predominantly of vascular origin. This book will be informative and of practical value for all practitioners responsible for caring for the very many patients who experience erectile dysfunction in the setting of hypertension and cardiovascular disease.

The understanding of the pathophysiology of erectile dysfunction (ED) has advanced significantly over the past two decades. In this ebook, we provide an in-depth analysis of the current knowledge of the pathophysiology of ED in order to provide the reader with an up-to-date and comprehensive understanding of the pathophysiology of ED, which is complex but fascinating. Intensive and intricate mechanisms govern the development of ED. These mechanisms involve complex interaction between vascular, hormonal and neural regulation. Current research on the central regulation of ED is key for the advancement of our understanding of this complex disease. Recent advances in our knowledge of the vascular mechanisms regulating penile erection led to appreciation of ED as an early symptom of cardiovascular disease. Major health concerns such as atherosclerosis, hyperlipidemia, hypertension and diabetes have become well integrated into the investigation of ED. Extensive research on the mechanisms involved in the regulation of penile erection have led to unparalleled advances in the understanding of general vascular regulation. However, many unknowns remain, which provides vast opportunities for future basic and clinical investigations. If erectile dysfunction is an ongoing issue, however, it can cause stress, affect your self-confidence and contribute to relationship problems. Problems getting or keeping an erection can also be a sign of an underlying health condition that needs treatment and a risk factor for heart disease. But even as we age, there are still plenty of ways to stay in the game and enjoy it. That's as true of sex as it is of sports.Erectile dysfunction (ED) can occur for many reasons. Sometimes it is as simple as the side effect of a particular medication. But for roughly 75% of men, the cause is more complex. ED may result from vascular disease, neurological disease, diabetes, or prostate-related treatments or surgeries.Whether you currently suffer from ED or are hoping to sidestep this condition, try these tips to overcome ED for better health and a better sex life.Start walking. According to one Harvard study, just 30 minutes of walking a day was linked with a 41% drop in risk for ED. Other research suggests that moderate exercise can help restore sexual performance in obese middle-aged men with ED.Eat right. In the Massachusetts Male Aging Study, eating a diet rich in natural foods like fruit, vegetables, whole grains, and fish - with fewer red and processed meat and refined grains - decreased the likelihood of ED.Pay attention to your vascular health. High blood pressure, high blood sugar, high cholesterol, and high triglycerides can all damage arteries in the heart (causing heart attack), in the brain (causing stroke), and leading to the penis (causing ED). An expanding waistline also contributes. Check with your doctor to find out whether your vascular system - and thus your heart, brain, and penis - is in good shape or needs a tune-up through lifestyle changes and, if necessary, medications.Size matters, so get slim and stay slim. A trim waistline is one good defense - a man with a 42-inch waist is 50% more likely to have ED than one with a 32-inch waist. Losing weight can help fight erectile dysfunction, so getting to a healthy weight and staying there is another good strategy for avoiding or fixing ED. Obesity raises risks for vascular disease and diabetes, two major causes of ED. And excess fat interferes with several hormones that may be part of the problem as well.Move a muscle, but we're not talking about your biceps. A strong pelvic floor enhances rigidity during erections and helps keep blood from leaving the penis by pressing on a key vein. In a British trial, three months of twice-daily sets of Kegel exercises (which strengthen these muscles), combined with biofeedback and advice on lifestyle changes quitting smoking, losing weight, limiting alcohol worked far better than just advice on lifestyle changes.

Erectile dysfunction is defined as having the inability to maintain an erection that is hard enough (stiffness) and that lasts long enough (duration) to achieve penetration during intercourse. It is characterized by the regular or repeated inability to obtain or maintain an erection. An occasional problem achieving an erection is nothing to worry about. But failure to do so more than 50% of the time at any age may indicate erectile dysfunction. The study of erectile dysfunction within medicine is covered by andrology, a sub-field within urology. Erectile dysfunction -ranging from complete inability to maintain erection to inadequate length of the erection - affects millions of men all over the world. Alone in the United States thirty million men suffer from erectile dysfunction.Erectile dysfunction is generally thought of as a condition only affecting older men, but a new study shows just how many young men may also have ED.A study showed that erectile dysfunction isn't being treated very often, with only 25.4 percent of men.Men with ED are also at higher risk for developing many diseases, including heart disease, arteriosclerosis (hardening of the arteries), diabetes mellitus, renal failure, liver and vascular disease. All these diseases have been associated with ED.Despite the launch of anti-impotence drugs, erectile dysfunction (ED) is still a taboo subject among men. Erectile Dysfunction (ED), is a subject that many men would prefer to ignore, not discuss with their partner or seek treatment for. Nearly a quarter of men are too embarrassed to even discuss it with their wives. Men wait an average of two years before seeking help for ED, during which time, any underlying conditions could get worse. The causes of ED, as it is commonly known, are varied. It can occur due to both physiological and psychological reasons.Despite the widespread opinion, that ED only is curable with chemical drugs, there are ways to overcome ED effective and permanently. And above all, in a very safe way. In this book you'll get introduced to one of the most powerfull weapons against ED.
